Empowering Health, Building Connections

Community Navigation

Community Navigation serves as a trusted link between individuals, families, and essential services. This role focuses on helping community members access healthcare, housing, food, employment, and public benefits by providing personalized support, resource connections, and ongoing follow-up.

Through outreach, relationship-building, and advocacy, the Community Navigator helps reduce barriers to care and improves overall well-being. The role also includes coordinating with partner organizations, tracking client progress, and contributing to data collection for program evaluation.

Housing Mediation Program

The Bridge is expanding its work to address one of the most pressing needs in our community: housing stability.

Our upcoming Housing Mediation Program is designed to support individuals, families, landlords, and community partners in navigating housing challenges before they escalate to the eviction process. Through guided conversations, resource connections, and conflict resolution, this program will create pathways for more stable and sustainable housing outcomes.

We recognize that housing issues are often complex and rooted in broader life circumstances. This initiative will focus on early intervention, helping to reduce barriers, strengthen communication, and promote solutions that benefit all parties involved.
